International Pet Travel Certificates in Charlotte, NC

Below you’ll find our International Pet Certificate Form, which you can complete at your convenience. Once submitted, our team will review the information and reach out to help schedule the appointments to complete any required services and obtain the appropriate travel documentation.

If your pet is new to our hospital, they will need to be seen for a consultation prior to the certificate appointment. This visit allows our medical team to review your pet’s health history, confirm required vaccines/tests, and answer any questions you may have regarding the travel certificate process. All travel-related questions will be addressed during that consultation.

There is a list of countries that allow the health certificate to be submitted electronically. That list can be found at https://www.aphis.usda.gov/aphis/pet-travel. If the destination country does not allow electronic endorsement:

  • Please obtain a prepaid express / overnight shipping label with YOUR address in the TO and FROM section (for example, FedEx or UPS).
  • You will need to schedule the health certificate appointment with enough time for the certificate to be returned. Do NOT wait until the day before travel.
  • Please bring the shipping label with you to the appointment.
